Welcome to Universal Primary Care (Southern Tier Community Health Center Network). Thank you for considering us. At Universal Primary Care we genuinely care about you and your family!
Formally referred to as Southern Tier Community Health Center Network, to our community we are known as Universal Primary Care. As the only Federally Qualified Health Center in Cattaraugus, Allegany and McKean counties, we strive to provide high quality, primary care to all patients regardless of a patient’s ability to pay. We also take great pride in offering these services through a designated Health Center model. Two names, one purpose: To offer every patient that best care possible.
Our “family” of practitioners, nurses, receptionists, billing and back office staff focuses daily on how to create a model of care that respects you, provides you great care and is compassionate and fair. It’s important to feel safe and confident when accessing health care. To feel good, stay healthy and be able to financially afford good care strikes at our most basic needs. That’s why we do what we do. Inside our walls, you will find a staff dedicated to mission, an organization committed to providing culturally sensitive, compassionate care and a model that attempts to meet you – the patient – where you are and help you navigate the complex world of health care.

It is our strongest belief that we are a part of the communities we serve. With sites in Olean, Cuba, Salamanca and Houghton, NY, as well as our new site in Bradford, PA we want to be more than a doctor’s office. We want to be your community health center. That means within our walls, we will not only provide you with great care, educational opportunities to self-manage your health status and access to a wide variety of social services but, we also want to be active in those communities we are lucky to serve. We get involved in projects that mean a healthier built environment for all of us.

Level 3 Patient-Centered Medical Home

Universal Primary care is committed to high-quality patient-centered care. Our nationally recognized quality efforts aim to provide our patients with not only the safest, highest quality of care, but a singularly great patient experience. Universal Primary Care is nationally recognized as a Level 3 Patient-Centered Medical Home. The designation is the highest level possible, focusing on building relationship-based health-care and comprehensive and collaborative services in our communities.

Community Health Quality Recognition

The Community Health Quality Recognition (CHQR) badges recognize Health Center Program awardees and look-alikes (LALs) that have made notable quality improvement achievements in the areas of access, quality, health equity, and health information technology for the most recent UDS reporting period. In addition, COVID-19 badges were introduced to recognize health centers’ contributions to the public health emergency response in the areas of data collection, testing, and vaccinations.
