Insurance & Financial Assistance

Universal Primary Care offers a Sliding Fee Scale to patients who are unable to pay for their medical or dental care due to financial barriers. The Sliding Fee scale can reduce your cost for services by between 10% and 100% and will be applied to all services rendered at UPC. Eligibility and slide level are based solely on your income and family size. To determine if you are eligible for a discount for services please review our sliding fee scale or contact our office at (716) 375-7500 and ask to speak to a Care Coordinator.
To apply for a sliding fee, you will need to complete an Application for Financial Assistance and return it to our office with proof of income for a three month period. Examples of proof of income include: a copy of your paystubs (3 months worth), unemployment determination letter, Social Security statement, or a copy of your previous years tax return. You do not needs to fill out a separate application for each member of your family but you will need to provide documentation of income for each family member (if applicable). If do not have any income to report you will need to complete and sign a No Income Attestation Form.
Once a determination has been made, you will receive an Eligibility and Determination letter from UPC stating if you were approved and your slide amount. You will also be provided a renewal date. Because an individual’s income and the NYS Poverty Level may change, you will need to reapply annually for our Sliding Fee Scale.

Universal Primary Care has several Certified Application Counselors that can assist individuals with enrolling in health insurance through the New York State of Health Marketplace. This service is offered to all UPC patients as well as any individual in the community who needs assistance with enrollment.

SPECIAL ENROLLMENT PERIOD

Individuals may be eligible to enroll in health insurance outside the Open Enrollment Period if they experience a qualify life event. Examples of a qualifying life event are:
  • Loss of health insurance (other than non-payment)
  • Move into or within the State of New York
  • Marriage or Death of a partner
  • Loss of insurance due to divorce
  • Birth or Adoption of a child
  • Change in eligibility for financial assistance
  • Become a citizen
  • No longer eligible to receive insurance under parent’s coverage

MEDICAID RECIPIENTS

Individuals can enroll in Medicaid anytime throughout the year. If you think you may be eligible to enroll in Medicaid and are interested in enrolling please contact our office to schedule an appointment with one of our Certified Application Counselors.
